I hereby fully understand that I am being hired on a 60-day probation period. In this time, if at any point my performance is subpar with regards to the expectations of my job description or I prove unable to meet the necessary expectations, that my employment will be terminated with or without notice at any time. This job description includes complying with uniform policies, which includes but are not limited to standard BKB Enterprises, LLC uniform and hygiene, being prepared to work at scheduled times, not having unapproved absences without doctor and/or management approval, ensuring the highest customer satisfaction, having customer awareness and meeting all training point requirements. This also includes avoiding any insubordination, management reprimands, as well as any type of violation of health code and/or safety policies, and completing tasks that I am assigned to within a given period of time and in their entirety.
I fully acknowledge that this agreement supersedes all other agreements contrary to those written or verbal, including the “At Will” Employment Statement for the trial period, which I will be hired on.
I understand that upon my initial hiring, the availability I claimed on my job application will be input on my behalf and must remain during this 60-day probation period. Once the probation period has passed, I will have the opportunity to alter my availability. If however, during this time frame I consistently shift pool or request off the shifts I claimed, I risk further continued employment. Upon completion of my probation period, I understand that any significant decrease in availability during high traffic sessions like Friday-Sunday may result in the loss of my employment.
I further acknowledge that this provision has been explained to me in a language that I understand. If I am unable to understand the language as written above, I acknowledge that I have received an explanation with a member of upper management or staff prior to signing this form.
